One of the most common questions I am asked as an instructor and also when I was precepting a million new nurses (ok, maybe not that many) - "What's the best way to take report - they talk so fast??!?"
Personally, I had developed my own little system - maybe I'll share it with you one day if you ask nice. But, I also point them to all the great resources on Google Images. You can find countless report sheets - you just need to find the one that works for you and for the population you are working with. The key is to have quick access to information that you may need as your day progresses.
